Bovine colostrum supplements are made by collecting that early milk from dairy cows after calves have fed, pasteurizing it, and spray-drying it into a powder. A typical commercial product contains:

  • Immunoglobulin G (IgG): the dominant antibody class, ~70–80% of total immunoglobulins in colostrum. Binds pathogens in the gut lumen and prevents adhesion to the intestinal wall (Playford & Weiser, Nutrients 2021).
  • Lactoferrin: an iron-binding glycoprotein that starves pathogens of iron and modulates immune signaling.
  • Growth factors: IGF-1 (insulin-like), TGF-β (transforming), and EGF (epidermal). IGF-1 promotes intestinal epithelial repair after injury; TGF-β supports tight-junction integrity and dampens inflammation; EGF accelerates mucosal regeneration (Playford & Weiser 2021).

Those components are why colostrum is biologically plausible for gut-barrier repair. They’re also why most pasteurized commercial yogurt and infant formula do not deliver the same effect: ordinary milk has only trace amounts of these molecules.

Why it’s everywhere right now

Search interest exploded between 2023 and 2026. US consumers spent more than $19 million on colostrum supplements in the 52 weeks ending January 3, 2026, a roughly 3,000% increase from two years prior, per industry tracking summarized by Nutritional Outlook. Celebrity endorsements (Jennifer Aniston’s morning routine, viral TikTok influencer videos) and brands like ARMRA accelerated the curve.

That commercial momentum is part of why the evidence-vs-hype gap matters. When a supplement quadruple-digit-scales its market in 24 months, the marketing inevitably outruns the science. With colostrum, the underlying biology is real; the leap to “fix everything from leaky gut to glowing skin” isn’t.

The strongest evidence: gut-barrier support under stress

The clearest randomized-trial signal sits in three contexts where the gut barrier is demonstrably damaged:

1. Exercise-induced intestinal permeability in athletes

Vigorous endurance training increases gut permeability acutely; runners commonly experience the resulting GI symptoms (“runner’s gut”). A landmark 2017 RCT randomized 16 trained athletes to 500 mg/day of bovine colostrum or placebo for 20 days during peak competitive training. The colostrum arm showed reductions in the urinary lactulose/mannitol ratio and stool zonulin concentration, both standard markers of paracellular permeability (Hałasa et al., Nutrients 2017).

The 2024 meta-analysis pooled 10 such RCTs and confirmed the effect across athletes and selected patient groups: colostrum significantly lowered lactulose/rhamnose and lactulose/mannitol ratios versus placebo. The authors flagged heterogeneity in dose and duration as a limitation (Halasa et al. 2024).

2. Infectious and antibiotic-associated diarrhea

The 2024 systematic review covering 22 trials and 1,427 patients reported that diarrhea frequency dropped with colostrum supplementation in 15 of 20 interventional arms that evaluated it, with the strongest signal in infectious diarrhea (rotavirus, Cryptosporidium) and in HIV-associated diarrhea (Cabinian et al. 2024). The mechanism is plausible: IgG in colostrum binds enteric pathogens directly, and the growth factors accelerate enterocyte repair.

3. NSAID-induced gut damage

NSAIDs (ibuprofen, naproxen, aspirin) reliably damage the small-bowel mucosa, even at over-the-counter doses. An early controlled trial in healthy volunteers found bovine colostrum prevented the increase in gut permeability caused by indomethacin compared to placebo (Playford et al., Clin Sci 2001). It’s one of the most replicated colostrum findings and is the basis for some clinical use in chronic NSAID users.

Where the evidence runs thin

Three claims dominate TikTok and most don’t yet have the trials to back them:

  • “Heals leaky gut” in the general population. The intestinal-permeability evidence is strongest in acute damage (exercise, NSAIDs, surgery, critical illness). Whether daily colostrum meaningfully changes baseline permeability in a healthy person without those stressors is not well-studied, and the “leaky gut syndrome” wellness framing remains contested in mainstream gastroenterology.
  • Skin, hair, anti-aging benefits. Most claims trace back to the same gut-barrier studies plus small open-label trials. There are no large RCTs showing visible skin or hair changes from oral colostrum.
  • Immune boosting in healthy adults. Trials in athletes show modest reductions in upper respiratory infection days during heavy training. Generalizing that to “fewer colds” in the general population stretches the data.

Cleveland Clinic’s recent take is blunt: most consumer-facing claims rest on small studies, mixed effectiveness, and doses higher than what’s available retail (Cleveland Clinic, Health Essentials). That’s the practical honesty most marketing skips.

How much, and how often

There is no consensus dose. Trials have used anywhere from 500 mg/day to 20 g/day:

Safety, allergies, and who should skip it

For most healthy adults, bovine colostrum is well-tolerated. Side effects are typically mild and limited to gas, bloating, or loose stool when first starting. But there are real reasons to skip it:

  • Cow’s milk allergy. Colostrum is concentrated dairy. People with IgE-mediated milk allergy can react severely; lactose-intolerant people may also have GI symptoms even though the lactose load per scoop is modest.
  • Pregnancy and lactation. Not well-studied; avoid without clinician input.
  • Severely immunocompromised or central-line patients. As with other live or biologically active supplements, talk to a clinician before use.
  • Vegan or strict vegetarian. Colostrum is an animal product. Plant-based “colostrum-like” mimics (egg-yolk antibody powders or synbiotic blends) are not pharmacologically equivalent.

Sourcing matters more here than for most supplements. The IgG content in commercial bovine colostrum products varies widely (4–40%), and US bovine colostrum is not regulated as a drug. Look for third-party testing (NSF, USP, ConsumerLab) and a stated IgG percentage on the label.

Where colostrum fits in the bigger gut-health picture

Bovine colostrum is one of three loosely related supplement categories that confuse consumers: probiotics (live microbes), prebiotics (fibers that feed them), and bovine colostrum (a mix of antibodies, growth factors, and prebiotic-like oligosaccharides).
